By enrolling and participating in this course you acknowledge and accept the following terms and conditions:

All physical manipulations of the body can be potentially hazardous. Attendance at this Seminar requires Hands-On training with other participants.  In registering online I personally and in full mental cogniscence agree to, the below waiver, release and non disclosure: release Evan Pantazi, Fred Mastison, Kyusho International Inc. all Instructors and agencies associated with this event in any capacity, from any and all liability, that may incur as a result of my willing participation at this event. I also release to Kyusho International Inc, the permission to use my image in any video or picture reproductions.

I understand that by registering online I agree to these Conditions.

Release

Also in consideration of my engagement as a model, upon the terms herewith stated, I hereby give to Kyusho International ® it’s officers, legal representatives and assigns, those for whom Kyusho International ® is acting, and those acting with his/her authority and permission:

a) the unrestricted right and permission to copyright and use, re-use, publish, and republish photographic portraits, pictures or video representations of me or in which I may be included intact or in part, composite or distorted in character or form, without restriction as to changes or transformations in conjunction with my own or a fictitious name, or reproduction hereof in color or otherwise, made through any and all media now or hereafter known for illustration, art, promotion, advertising, trade, or any other purpose whatsoever.

b) I also permit the use of any printed material in connection therewith.

c) I hereby relinquish any right that I may have to examine or approve the completed product or products or the advertising copy or printed matter that may be used in conjunction therewith or the use to which it may be applied.

d) I hereby release, discharge and agree to save harmless to Kyusho International ® it’s officers, legal representatives and assigns, those for whom Kyusho International ®, and all persons functioning under his/her permission or authority, or those for whom he/she is functioning, from any liability by virtue of any blurring, distortion, alteration, optical illusion, or use in composite form whether intentional or otherwise, that may occur or be produced in the taking of said picture or in any subsequent processing thereof, as well as any publication thereof, including without limitation any claims for libel or invasion of privacy.

e) I hereby affirm that I am over the age of majority and have the right to contract in my own name. I have read the above authorization, release and agreement, prior to its execution; I fully understand the contents thereof. This agreement shall be binding upon me and my heirs, legal representatives and assigns.  Any and all litigation will be held and or prosecuted under and in the legal jurisdiction of the Commonwealth of Massachusetts, USA.

Non Disclosure

WHEREAS, Kyusho International® and myself as applicant, wish to exchange, for the purposes of evaluating a business proposal hereinafter set forth, certain proprietary company-confidential information pertaining to their respective business and expertise, and wish to provide for the protection of the confidentiality of any such information provided by one to the other. The Receiving Party will hold such proprietary company-confidential information in confidence; will use such information only for the purposes prescribed in the licensing agreement  presented by Kyusho International®

 The Receiving Party agrees to accept the terms and conditions herein by registering and does hereby state and agree that they will not disclose any materials or information to any individuals or groups, that they receive from Kyusho International®  or from any of it’s members or officers. The Receiving Party  further agree not to use any information gained from the disclosure of information in respect to the instructional license privileges being presented by Kyusho International®  to compete with Kyusho International®  and or any members or affiliates for a period of 10 years after the acceptance of the terms and agreements stated here and in the records of Kyusho International®  .

It is also understood that The Receiving Party will be privileged to copyrighted materials, information, graphics, business plans, policies and procedures and will not disclose or make use of these stated properties of Kyusho International®  unless given written consent from Evan Pantazi, President and CEO of Kyusho International® .

You must order and train this program prior to attending this Certification Course.
